Transition Assessment 

Empathetically Educating places a strong emphasis on self-determination and the dignity of risk throughout the transition assessment process. These assessments are designed to support students and their families as they navigate the transition from school to adult life and future opportunities.

The assessment process involves gathering information across a variety of domains, including employment, independent living, recreation, community engagement, and post-secondary education. By exploring each individual’s strengths, needs, preferences, and interests, transition assessments help guide the development of meaningful goals and services within the Individualized Education Program (IEP).

Consultation

In addition to transition assessments, our services also include consultation. Transitioning into adulthood can feel overwhelming for individuals and families trying to understand the many agencies, programs, and support systems available. As part of our consultation services, we help families navigate the adult service landscape with greater clarity and confidence.We provide guidance in understanding available supports, eligibility requirements, application processes, and long-term planning options related to adult services. This may include assistance with:

• Transition planning after high school

• Day programs and vocational services

• Independent living and life skills supports

• Guardianship and decision-making resources

• Benefits and funding resources

• Connecting with local programs and providers

• Building a roadmap for long-term support needs
