Transition Assessment
Empathetically Educating places a strong emphasis on self-determination and the dignity of risk throughout the transition assessment process. These assessments are designed to support students and their families as they navigate the transition from school to adult life and future opportunities.
The assessment process involves gathering information across a variety of domains, including employment, independent living, recreation, community engagement, and post-secondary education. By exploring each individual’s strengths, needs, preferences, and interests, transition assessments help guide the development of meaningful goals and services within the Individualized Education Program (IEP).
