Transition Assessment 

Empathetically Educating places a strong emphasis on self-determination and the dignity of risk in our transition assessments with clients. These assessments aim to facilitate students and their families in navigating the transition from school to subsequent endeavors. The evaluation process encompasses gathering pertinent information across various domains such as employment, independent living, recreation, community engagement, and post-secondary education. By delving into individuals' needs, preferences, and interests, these assessments lay the groundwork for establishing goals and services outlined in Individualized Education Programs (IEPs). Transition assessments encompass a range of evaluations including behavioral assessments, aptitude tests, interest and work values inventories, intelligence and achievement tests, personality assessments, career readiness evaluations, self-determination assessments, work-related temperament assessments, and transition planning inventories.
